Cardida Contest Rules and Disclaimer

The Rules.

A. Entry: No purchase or online entry is necessary. This sweepstakes is open to all legal residents of the United States, who are 21 years of age or older; except employees of Cardida Corporation, its affiliates, parent companies, participating vendors, subsidiaries and advertising and promotion agencies and the immediate family members or those living in the same household of each. Void in NY, Florida, Rhode Island and wherever prohibited or restricted by law. Subject to federal, state and local laws. Limit one entry per person. Entrants must be at least 21 years of age and entries must be filled in completely in order to qualify for this or any other follow on promotions or drawings. Some entrants will be entered into other drawings or promotions at which time telephone or mail notification of results or opportunities will be made. Other awards by this or other sponsors, will be subject to the terms and conditions of that game, contest ot drawing and will be appropriately disclosed. By completing this form, you authorize Cardida Corporation to remove your name from our DO NOT CALL list. To enter online, complete an official entry form located on the main Sweepstakes page. The Sweepstakes begins January 1, 1999 and ends December 24, 2000. All online entries must be received by 11:59 PM ET on December 24, 2000. To enter by mail, send a 3 x 5 card on which you have printed your Name, Address City, State Zip, Email Address (if applicable) Birth date, Gender, Marital Status, Home Owner Status, Income and Wether you are Retired or Not to: Cardida Corporation, Millenium Blazer Giveaway, 10150 East Pike Road Cambridge, Oh. 43725. Mail-in entries must be postmarked by December 24, 2000 and received by January 6, 2001. This is an advertising promotion of multiple Sponsors and is void where prohibited by law. The odds of winning are unknown and cannot be determined due to the unknown number of entrants thru this website as well as by mail. LIMIT ONE ENTRY PER PERSON, REGARDLESS OF MEANS OF ENTRY.

No responsibility is assumed by Cardida Corporation or Sponsors for lost, late, misdirected, illegible, or mutilated entries or for any computer, online, telephone or technical malfunctions that may occur. Entries will be declared made by the authorized account holder of the e-mail address submitted at the time of entry. "Authorized account holder" is defined as the natural person who is assigned to an email address by an Internet Access provider, on-line service provider, or other organization (e.g., business, educational institution etc.) that is responsible for assigning email addresses for the domain associated with the submitted email address. All entries become the property of Cardida Corporation and their Sponsors and will not be returned to entrant. If the potentially winning entry is an Internet or other computer entry, potential winner may be requested to provide their Sponsors with proof that the potential winner is the authorized account holder of the email address associated with the winning entry.

B. Winner Selection: Prize winners will be selected in a random drawing to be held on or about January 24, 2001 by the Cardida Corporation. Winners will be notified by email and/or phone within 30 days of selection. If winner is unreachable after 30 days, or if the winner is unavailable for prize fulfillment, an alternate winner will be selected at random from all remaining eligible entries received.

C. Prizes: One (1) prize winner may select either option 1 or 2:

1. A Millenium Model (2000) Chevy Blazer. Cardida Corporation will pay for the Blazer only. Winner is responsible for all other costs (including applicable taxes, license title and insurance) Estimated retail value: $21,000 - 25,000.

2. A cash award of $25,000.

No substitution or transfer of prizes is permitted except by Cardida Corporation and their Sponsors. Cardida Corporation and their Sponsors reserve the right to substitute the prize with another prize of equal or greater value.

D. Eligibility: This sweepstakes is open to legal residents of the United States, who are eighteen (21) years or older at the time of entry. Void in Puerto Rico, New York, Rhode Island and Florida. Void to all Florida residents. Void where prohibited by Federal, State or Local law. Employees of Cardida Corporation and their Sponsors, their respective parents, their affiliates, subsidiaries, advertising and promotional agencies and the immediate family members of each are not eligible.

E. General Conditions: Sweepstakes entrants agree to abide by the terms of these Official Rules and by the decisions of the judging organization which are final on all matters pertaining to the Sweepstakes. This Sweepstakes is governed by the laws of the United States and the State of Ohio. All federal, state and local laws and regulations apply. All taxes, fees, and surcharges on prizes are the sole responsibility of the prize winner. Return of any prize or prize notification as undeliverable may result in disqualification and selection of an alternate winner.

Winners further agree to grant to Cardida Corporation and their Sponsors the right to use and publish their proper name, state and pictures online and in print, or any other media, in connection with this Sweepstakes and for future promotions. Acceptance of a prize constitutes permission for Cardida Corporation, Sponsors and their advertising and promotional agencies to use winners names and likenesses for advertising and promotional purposes without additional compensation unless prohibited by law. By entering, participants release and hold harmless Cardida Corporation, Sponsors, their respective parents, subsidiaries, affiliates, directors, officers, employees, advertising agencies, web masters and agents from any and all liability for any injuries, loss or damage of any kind arising from or in connection with the Sweepstakes or any prize won. Restrictions, conditions, and limitations apply.

F. Conduct: By entering this Sweepstakes, you agree to be bound by these Official Rules. The Official Rules will be posted at the Sweepstakes Site throughout the Sweepstakes. Entrants further agree to be bound by the decisions of the judges, which shall be final and binding in all respects. Cardida Corporation reserves the right at its sole discretion to disqualify any individual it finds to be tampering with the entry process or the operation of the game or web-site; to be acting in violation of the Official Rules; or to be acting in an un-sportsmanlike or disruptive manner, or with intent to annoy, abuse, threaten or harass any other person. CAUTION: ANY ATTEMPT BY A CONTESTANT OR ANY OTHER INDIVIDUAL TO DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE GAME IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, CARDIDA CORPORATION RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH CONTESTANT TO THE FULLEST EXTENT PERMITTED BY LAW.

G. Limitations of Liability: Cardida Corporation and their Sponsors are not responsible for any incorrect or inaccurate information, whether caused by web-site users or by any of the equipment or programming associated with or utilized in the Sweepstakes or by any technical or human error which may occur in the processing of submissions in the Sweepstakes. Cardida Corporation and their Sponsors assume no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or authorized access to, or alteration of, entries. Cardida Corporation and their Sponsors are not responsible for any problems or technical malfunction of any telephone network or lines, computer on-line-systems, servers or providers, computer equipment, software, failure of email or players on account of technical problems or traffic congestion on the Internet or at any web-site or combination thereof, including injury or damage to participants or to any other person's computer related to or resulting from participating or downloading materials in this Sweepstakes. If, for any reason, the Sweepstakes is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of Cardida Corporation and their Sponsors which corrupt or affect the administration, security, fairness, integrity or proper conduct of this Sweepstakes, Cardida Corporation and Sponsors reserve the right at their sole discretion to modify the Sweepstakes.
